Community Health Nurse

We are seeking a highly motivated and experienced Community Nurse to join our Coastal Community Team. As a Community Nurse, you will be responsible for providing high-quality, patient-centered care to individuals in their own homes. You will work closely with a multidisciplinary team to assess, plan, implement, and evaluate patient care, and develop care plans that meet the individual needs of each patient.
About the RoleThis is an exciting opportunity to work in a dynamic and supportive team environment, where you will have the opportunity to develop your skills and expertise in community nursing. You will be supported by a senior team of nurses and a clinical lead nurse, and will have access to ongoing training and development opportunities.
Key Responsibilities- Assess, plan, implement, and evaluate patient care, and develop care plans that meet the individual needs of each patient.
- Work closely with a multidisciplinary team to provide high-quality, patient-centered care.
- Develop and maintain relationships with patients, carers, and other healthcare professionals to ensure seamless care coordination.
- Conduct holistic assessments of patients, identifying their physical, emotional, and social needs.
- Develop and implement individualized care plans that address the unique needs of each patient.
- Provide education and support to patients and carers on health-related topics, including disease management and self-care.
- Collaborate with other healthcare professionals to ensure comprehensive care and support for patients.
- Participate in multidisciplinary team meetings to discuss patient care and develop care plans.
- Registered Nurse qualification and current NMC registration.
- Experience of working in a community setting, preferably in a similar role.
- Excellent communication and interpersonal skills, with the ability to work effectively with patients, carers, and other healthcare professionals.
- Ability to work independently and as part of a team, with a high level of autonomy and flexibility.
- Strong problem-solving and critical thinking skills, with the ability to analyze complex situations and develop effective solutions.
- Ability to maintain accurate and detailed records, and to use electronic patient records systems.
