Community Health Nurse

We are seeking a highly motivated and experienced Community Health Nurse to join our team at Norfolk Community Health and Care NHS Trust. As a Community Health Nurse, you will play a vital role in providing high-quality, patient-centered care to individuals in their own homes.
Key Responsibilities- Assess, plan, implement, and evaluate patient care, developing individualized care plans that meet the unique needs of each patient.
- Conduct holistic assessments, communicating complex information to patients and their families, and devising evidence-based care programs.
- Liaise with GPs, Social Services, and other healthcare professionals to ensure seamless care coordination.
- Participate in multidisciplinary meetings, contributing to the development of care plans and ensuring patient needs are met.
- Ensure accurate and timely documentation of patient records, utilizing electronic systems to input data.
- Work collaboratively with the team to achieve service targets and patient outcomes.
- Current registration with the Nursing and Midwifery Council (NMC).
- Experience working in a clinical setting, with a strong understanding of community nursing principles.
- Excellent communication and interpersonal skills, with the ability to adapt communication styles to suit different situations.
- Ability to work independently, with a high degree of autonomy, and as part of a team.
- Basic IT skills, with proficiency in electronic systems and software.
