Community Healthcare Worker

We are looking for a highly motivated Community Healthcare Worker to fill a Named Role within our Transformation Unit in Sheffield. In this role, you will work closely with patients to deliver high-quality care and support, using your expertise in mental health nursing or social work to make a positive impact.

Key Responsibilities:- Develop and implement individualized plans of care for patients
- Assess and manage clinical risk, using evidence-based practices to inform decision-making
- Collaborate with multidisciplinary teams to ensure seamless care coordination
- Provide education and training to colleagues and patients
- Contribute to quality improvement initiatives
This is a full-time position offering a salary of £43,500 per annum, plus a range of benefits including flexible working arrangements and access to ongoing training and development opportunities.
