- Assess the needs and risks of service users, identifying appropriate signposting and support services to promote their well-being and independence.
- Develop and deliver small group and activity sessions to support the needs of service users, promoting social connections and community engagement.
- Respond to the needs of individuals with sensitivity, respecting their age, culture, race, gender, ethnicity, social class, or disability.
- Support individuals to manage their own healthcare, in accordance with their care plan, to prevent further crises.
- Signpost service users to local community resources to address cultural or social barriers affecting their well-being.
- Link service users with specialist services for continuous support, such as counseling, money management, or substance misuse.
- Promote the rights of individuals, recognizing differences and acting in accordance with relevant legislation.
- Promote effective communication with people to build positive working relationships with individuals, teams, and agencies.
- Contribute to the protection of individuals from abuse and harm, recognizing and reporting any signs of neglect or harm.
- Ability to rapidly form and sustain trusting relationships with service users and their families.
- Well-developed listening skills and effective verbal communication skills to engage with service users, carers, and colleagues.
- Lived experience of the acute mental health care pathway, such as emergency department attendance or acute admissions.
- Knowledge of recovery tools, mental health, and wellbeing support services available in the local area.
- Ability to assess needs, risks, and aspirations, identifying appropriate signposting and support services.
- Good IT skills, including a working knowledge of Microsoft Office and social media platforms.
- Knowledge and understanding of safeguarding issues and ability to address them appropriately.
- Able to demonstrate a general understanding of mental health issues with a positive, non-judgmental approach towards individuals in crisis.
