This is an exciting opportunity to join our team as an Advanced Nurse Practitioner in the East Kennet Primary Care Network. We are seeking a highly skilled and experienced nurse to work in a dynamic and supportive environment.
Job SummaryThe successful candidate will be responsible for providing high-quality patient care, working autonomously to assess, diagnose, and treat patients with undifferentiated, undiagnosed problems. They will also be responsible for implementing and evaluating individualized treatment plans for chronic disease patients, identifying and managing patients at risk of developing long-term conditions, and providing advanced nursing care to patients as required.
Main Duties and Responsibilities- Manage patients presenting with a range of acute and chronic medical conditions, providing subject matter expert advice.
- Implement and evaluate individualized treatment plans for chronic disease patients.
- Identify, manage, and support patients at risk of developing long-term conditions, preventing adverse effects to the patient's health.
- Provide advanced, specialist nursing care to patients as required in accordance with clinical-based evidence, NICE, and the NSF.
- Undertake the collection of pathological specimens, request pathology services as necessary, process and interpret pathology and other test results as required.
- Maintain accurate clinical records in conjunction with extant legislation, ensure SNOMED CT codes are used effectively, and chaperone patients where necessary.
- Prioritize health issues and intervene appropriately, support the team in dealing with clinical emergencies, recognize, assess, and refer patients presenting with mental health needs.
- Be an extended and supplementary prescriber, adhering to extant guidance, support patients in the use of their prescribed medicines or over-the-counter medicines (within own scope of practice), reviewing annually as required.
- Contribute to practice targets (QOF, etc.), complying with local and regional guidance, liaise with external services/agencies to ensure the patient is supported appropriately (vulnerable patients, etc.).
- Delegate clinical responsibilities appropriately, ensuring safe practice and that the task is within the scope of practice of the individual, support the clinical team with all safeguarding matters in accordance with local and national policies.
- Understand practice and local policies for substance abuse and addictive behavior, referring patients appropriately, deliver opportunistic health promotion where appropriate.
- Enhance own performance through continuous professional development, keep up to date with current evidence-based practice, and impart own knowledge and skills to PCN colleagues to meet the needs of the service.
- Be verified against the ANP roadmap, recognize and work within own competence and professional code of conduct as regulated by the NMC.
- Assess effectiveness of care delivery through self and peer review, benchmarking, and formal evaluation, participate in research and utilize the audit cycle as a means of evaluating the quality of the work of self and the team, implementing improvements where required.
- Produce accurate, contemporaneous, and complete records of patient consultation, consistent with legislation, policies, and procedures.
- Be aware of data protection (GDPR) and confidentiality issues particularly within a PCN, use technology and appropriate software as an aid to management in planning, implementation, and monitoring of care, presenting and communicating information.
- Review and process data using accurate read codes in order to ensure easy and accurate information retrieval for monitoring and audit processes.
The postholder will have access to appropriate clinical supervision and an appropriate named individual in the PCN to provide general advice and support on a day-to-day basis.
