Job Summary

This is an exciting new Advanced Nurse Practitioner role, working for the East Kennet Primary Care Network and based at Kennet & Avon Medical Partnership in Marlborough and Pewsey. The role will be predominately focused on same day, medically urgent patients.

Main Duties of the Job

Advanced Nurse Practitioners work autonomously within the practice using their enhanced clinical assessment and treatment skills, to provide first point of contact for patients presenting with undifferentiated, undiagnosed problems relating to minor illness or injury, abdominal pains, chest pains and headaches. They are autonomous in making decisions based on assessment, diagnosis and interpretation of test results. They can independently prescribe appropriate medication, evaluate or refer to other specialists if necessary. ANPs in this role will focus solely on medically urgent patients.

About the Role

This role will join a team of paramedics, nurse practitioners and duty GPs who are responsible for treating same day, medically urgent patients. The role will be based from either site and will also include some home visits.

Key Responsibilities

  • Manage patients presenting with a range of acute and chronic medical conditions, providing subject matter expert advice
  • Implement and evaluate individual specialised treatment plans for chronic disease patients
  • Identify, manage and support patients at risk of developing long-term conditions, preventing adverse effects to the patient's health
  • Provide advanced, specialist nursing care to patients as required in accordance with clinical based evidence, NICE and the NSF
  • Undertake the collection of pathological specimens
  • Request pathology services as necessary
  • Process and interpret pathology and other test results as required
  • Maintain accurate clinical records in conjunction with extant legislation
  • Ensure SNOMED CT codes are used effectively
  • Chaperone patients where necessary
  • Prioritise health issues and intervene appropriately
  • Support the team in dealing with clinical emergencies
  • Recognise, assess and refer patients presenting with mental health needs
  • Be an extended and supplementary prescriber, adhering to extant guidance
  • Support patients in the use of their prescribed medicines or over the counter medicines (within own scope of practice), reviewing annually as required
  • Contribute to practice targets (QOF, etc.), complying with local and regional guidance
  • Liaise with external services/agencies to ensure the patient is supported appropriately (vulnerable patients, etc.)
  • Delegate clinical responsibilities appropriately (ensuring safe practice and that the task is within the scope of practice of the individual)
  • Support the clinical team with all safeguarding matters in accordance with local and national policies
  • Understand practice and local policies for substance abuse and addictive behaviour, referring patients appropriately
  • Deliver opportunistic health promotion where appropriate

  • Be verified against the ANP roadmap
  • Recognise and work within own competence and professional code of conduct as regulated by the NMC
  • Assess effectiveness of care delivery through self and peer review, benchmarking and formal evaluation
  • Participate in research and utilise the audit cycle as a means of evaluating the quality of the work of self and the team, implementing improvements where required

Administrative Requirements

  • Produce accurate, contemporaneous and complete records of patient consultation, consistent with legislation, policies and procedures
  • Be aware of data protection (GDPR) and confidentiality issues particularly within a PCN
  • Use technology and appropriate software as an aid to management in planning, implementation and monitoring of care, presenting and communicating information
  • Review and process data using accurate read codes in order to ensure easy and accurate information retrieval for monitoring and audit processes

Supervision

The postholder will have access to appropriate clinical supervision and an appropriate named individual in the PCN to provide general advice and support on a day-to-day basis.
