Advanced Nurse Practitioner Role

This is an exciting opportunity to work as an Advanced Nurse Practitioner in the East Kennet Primary Care Network. The successful candidate will be based at Kennet & Avon Medical Partnership in Marlborough and Pewsey, working predominately with medically urgent patients.
Main DutiesThe Advanced Nurse Practitioner will work autonomously within the practice, using their enhanced clinical assessment and treatment skills to provide first point of contact for patients presenting with undifferentiated, undiagnosed problems relating to minor illness or injury, abdominal pains, chest pains, and headaches. They will be responsible for making decisions based on assessment, diagnosis, and interpretation of test results, and will be able to independently prescribe appropriate medication and evaluate or refer to other specialists as necessary.
The ANP will focus solely on medically urgent patients and will work a full day every Friday. They will be part of a team of paramedics, nurse practitioners, and duty GPs who are responsible for treating same-day, medically urgent patients. The role will involve some home visits and will require the ability to work effectively in a team and communicate with a range of stakeholders.
Key Responsibilities- Manage patients presenting with a range of acute and chronic medical conditions, providing subject matter expert advice.
- Implement and evaluate individualised treatment plans for chronic disease patients.
- Identify, manage, and support patients at risk of developing long-term conditions, preventing adverse effects on their health.
- Provide advanced, specialist nursing care to patients as required, in accordance with clinical-based evidence, NICE, and the NSF.
- Undertake the collection of pathological specimens, request pathology services as necessary, and process and interpret pathology and other test results as required.
- Maintain accurate clinical records in conjunction with extant legislation, ensuring SNOMED CT codes are used effectively.
- Chaperone patients where necessary, prioritise health issues, and intervene appropriately.
- Support the team in dealing with clinical emergencies, recognise, assess, and refer patients presenting with mental health needs.
- Be an extended and supplementary prescriber, adhering to extant guidance, and support patients in the use of their prescribed medicines or over-the-counter medicines (within own scope of practice), reviewing annually as required.
- Contribute to practice targets (QOF, etc.), complying with local and regional guidance, and liaise with external services/agencies to ensure the patient is supported appropriately.
- Delegate clinical responsibilities appropriately, ensuring safe practice and that the task is within the scope of practice of the individual.
- Support the clinical team with all safeguarding matters in accordance with local and national policies, and understand practice and local policies for substance abuse and addictive behaviour, referring patients appropriately.
- Deliver opportunistic health promotion where appropriate.
- Enhance own performance through continuous professional development, keeping up to date with current evidence-based practice, and impart own knowledge and skills to PCN colleagues to meet the needs of the service.
- Be verified against the ANP roadmap, recognise and work within own competence and professional code of conduct as regulated by the NMC.
- Assess effectiveness of care delivery through self and peer review, benchmarking, and formal evaluation, and participate in research and utilise the audit cycle as a means of evaluating the quality of the work of self and the team, implementing improvements where required.
- Produce accurate, contemporaneous, and complete records of patient consultation, consistent with legislation, policies, and procedures, and be aware of data protection (GDPR) and confidentiality issues particularly within a PCN.
- Use technology and appropriate software as an aid to management in planning, implementation, and monitoring of care, presenting and communicating information, and review and process data using accurate read codes in order to ensure easy and accurate information retrieval for monitoring and audit processes.
The postholder will have access to appropriate clinical supervision and an appropriate named individual in the PCN to provide general advice and support on a day-to-day basis.
