Job Title: Social Worker
Job Summary:
We are seeking a highly skilled and experienced Social Worker to join our team at Connect2Kent. As a Social Worker, you will be responsible for managing a caseload of disabled children and young people, assessing their needs, and developing personalized support plans to promote their independence and well-being.
Main Responsibilities:
- Manage a complex and diverse caseload of children, young people, and their families, undertaking assessments to determine their needs and capabilities.
- Develop and implement personalized support plans, working in collaboration with service users, their families, and other agencies.
- Provide advocacy and support to service users, empowering them to make informed decisions about their lives.
- Identify and manage safeguarding and quality of care issues, ensuring the well-being and safety of service users.
- Develop and maintain good working relationships with Education and Health staff, contributing to Education, Health, and Care plans.
- Research and stay up-to-date with changes in legislation, theories, and policies related to disabilities, disseminating knowledge throughout the team.
- Contribute to the development of new initiatives and best practices, attending working groups, training courses, and multi-agency forums.
- Maintain accurate electronic records and write reports for formal meetings, including reviews, case conferences, and court proceedings.
Requirements:
- Good interpersonal skills, with the ability to communicate effectively with clients, colleagues, and other agencies.
- Ability to prioritize and work effectively on own initiative, as well as within a team.
- Interest in leading training courses and supervising student placements.
- Supervisory, mediation, and negotiation skills.
- Good report-writing skills and the ability to communicate clearly in writing.
- Ability to work within a court setting.
- Computer literacy and ability to travel across a wide geographical area.
Knowledge and Skills:
- Thorough knowledge of human development, social work theories, and the needs of people with disabilities.
- Good knowledge of the legislation underpinning the provision of services to children and adults with disabilities.
- Working knowledge of Directorate and national policies, frameworks, and regulations.
- Knowledge of Child and Adult Protection procedures.
- Knowledge of court proceedings.
- Up-to-date knowledge of research.
- Knowledge of family relationships.
- Good knowledge of assessment frameworks.
