Social Care Professional

This is an exciting opportunity to contribute to the Short-Term Pathways Team in East Kent, promoting professional development and collaboration with health and partner agencies.
The team focuses on promoting independence for adults who have had a hospital stay or short-term care, working collaboratively to meet their future care and support needs.
We are looking for motivated professionals who are committed to embracing new ways of working, helping shape our services and promoting a person-centred approach.
As a social worker in this team, you will work with adults over 18 with diverse care and support needs, managing a caseload requiring complex assessment and creative care planning.
Through excellent opportunities for training and development, including a comprehensive induction, you will receive the support you need and recognition for your good work.
We are increasingly accessible to the public outside of traditional office hours, moving towards an 8am to 8pm, 7 days a week service to meet service needs.
These posts are linked to assessments of individuals in care homes across East Kent, with a work base discussed at interview. There is an expectation that the successful applicant will travel to various locations in East Kent to undertake Short-Term Pathways work.
