Job Overview:
A part-time opportunity has become available within the Transition team at Bolton at Home Limited, supporting young people aged 14-25 with special education needs and disabilities in their transition to adulthood.

About the Role:
This role requires a qualified social worker with an enthusiasm for working with vulnerable adults and children, ensuring a smooth transition process that addresses their current and future aspirations. The successful candidate will work collaboratively with both Children and Adult services, maintaining relationships with young people, their families, and key partners.

The ideal candidate will have excellent communication skills, enabling them to develop and maintain effective partnerships to support individuals in achieving their goals. Responsibilities:
The successful applicant will be responsible for discharging duties in accordance with the Care Act 2014, which includes supporting young people in preparing for adulthood. This involves developing a deep understanding of their needs, current and future aspirations, and providing tailored support to ensure a successful transition.
