Key Responsibilities
  • Work as part of a multidisciplinary team within the PCN to deliver high-quality patient care.
  • Assess and triage patients, including same-day triage, and provide definitive treatment or make necessary referrals to other members of the primary care team.
  • Provide health education and advice to patients, promoting self-management and signposting to social prescribing services.

The ideal candidate will have a BSc or above in Paramedic Science or equivalent degree, be registered with the HCPC, and have a strong understanding of clinical governance and equality and valuing diversity principles.
