Job Responsibilities

  • Broker personalized care packages and placements for adults, ensuring the provision of tailored support.
  • Negotiate with care providers to secure placements that meet individual needs and are within allocated budgets.
  • Facilitate timely discharge of patients from hospitals at weekends or outside normal working hours.
  • Liaise with social care practitioners to confirm required services and specified outcomes.

We seek an ideal candidate with:

  • An enhanced DBS check.
  • Experience working within Health and Social Care.
  • A full UK driving licence.

The estimated salary range for this position is £15,700-£19,100 per annum, depending on qualifications and experience. Work will be based at Grantham Hospital and primarily conducted Monday to Friday with occasional weekend work on a rota basis.
