Care Coordinator Specialist

We are seeking a highly skilled Care Coordinator Specialist to join our team at Service Care Solutions Ltd.
About the Role:
The ideal candidate will have experience of working within Health and Social Care, with a strong understanding of care packages and placements for adults.
You will be responsible for brokering care packages and placements, negotiating with care providers to secure placements that meet individual needs, and proactively facilitating the timely discharge of patients from hospitals.
About You:
To be successful in this role, you will require an Enhanced DBS check, experience of working within Health and Social Care, and a full UK driving licence.
Benefits:
This is a temporary, full-time position for an initial period of 3 months with a view to extending on a rolling basis. The pay rate for this role is £15.70 per hour.
