Community Care Coordinator

Service Care Solutions Ltd is seeking a Community Care Coordinator to develop and implement tailored care plans for individuals requiring ongoing support following discharge from hospital.
The role involves managing a caseload of service users, each with unique support needs, within allocated personal budgets. The successful candidate will work closely with care providers to secure placements that meet individual requirements.
Key responsibilities include:
- Broker care packages and placements for adults, ensuring personalized and tailored support.
- Negotiate with care providers to secure placements that meet individual needs and are within allocated personal budgets.
- Facilitate timely discharges from hospitals, working collaboratively with healthcare professionals and social care practitioners.
- Liaise with social care practitioners to confirm required services and specified outcomes.
The ideal candidate will possess:
- An Enhanced DBS check.
- Experience in Health and Social Care.
- A full UK driving licence.
Salary: £15.70 per hour. This is a temporary, full-time position for an initial period of 3 months, with the potential for extension on a rolling basis.
