Job Overview

We are seeking a highly skilled and compassionate Discharge Coordinator to join our team at University Hospitals Sussex NHS Foundation Trust. As a key member of our Discharge Team, you will play a vital role in ensuring the smooth transition of patients from hospital to home or other care settings.

Main Responsibilities
  • Complete Fastrack assessments for end of life patients
  • Attend daily huddles and board rounds to identify patients suitable for Fastrack
  • Participate in weekly multi-disciplinary team meetings on each ward in your caseload
  • Liaise with relatives, All Age Continuing Healthcare members, and community partners

Person Specification
  • Registered Nurse on the current NMC register or Qualified Health or Social Care Professional on the HCPC register
  • Essential criteria: Good oral communication skills, ability to empathise and reassure in stressful situations
  • Desirable criteria: Teaching and assessing, mentorship, experience in completing Fastracks
