About the Role

We are seeking a highly skilled and compassionate Physiotherapist to join our Specialist Palliative Care team at St Barnabas Hospices. As a key member of our multidisciplinary team, you will play a vital role in providing high-quality care to patients with complex needs.

Key Responsibilities
  • Assess, plan, implement, and evaluate treatment programs for patients to maintain and improve their functional abilities.
  • Develop and maintain effective relationships with patients, families, and healthcare professionals to ensure seamless care coordination.
  • Conduct comprehensive assessments, including home visits, to identify patient needs and develop personalized care plans.
  • Collaborate with the multidisciplinary team to ensure timely and effective discharge planning, including arranging ongoing support as needed.
  • Provide education and support to patients and their families to promote independence and self-management.
  • Participate in clinical and multidisciplinary meetings to ensure effective communication and coordination of care.
  • Maintain accurate and comprehensive records, adhering to relevant policies and procedures.
Requirements
  • Degree or Diploma in Physiotherapy
  • HCPC Registered Physiotherapist with at least 2 years of post-registration experience
  • Experience working with palliative care, oncology, neurology, community, or care of the elderly patients
  • Excellent communication and interpersonal skills
  • Ability to demonstrate empathy and compassion in challenging situations
