About the Role

This exciting opportunity has arisen for a motivated and enthusiastic individual to join our Reactive Emergency Assessment Community Team (REACT) as a Senior Registered Nurse.

We are seeking an experienced nurse who can deliver high-quality patient-focused care in the community. The successful candidate will be part of a multi-disciplinary team providing acute care services to patients in their homes, working collaboratively with other healthcare professionals.

The role involves travelling throughout East Suffolk, delivering care to patients during core hours and out-of-hours shifts. You will be expected to work 11-11 triage shifts, including weekends and bank holidays, and have access to pool cars for transportation. A valid driving license is essential.

Main Responsibilities
  • Deliver clinical expertise and patient care in partnership with OPAT teams
  • Assess and provide care to patients in crisis to prevent admission
  • Liaise with district nursing services and other community teams to ensure continuity of care
  • Coordinate case loads, schedule visits, and provide clinical supervision
Requirements
  • Registered Nurse qualification and post-registration training
  • Evidence of previous experience at Band 6 level
  • Sound knowledge of diabetes management and End of Life Care
  • Ability to prioritize clinical issues and manage risk

We offer a competitive salary of £37,338 - £44,962 per annum, pro rata, along with a generous pension scheme and 27 days annual leave on commencement.
