We are seeking a skilled and compassionate Complex Care Specialist to join our team at Home Group. As a Positive Behaviour Support Professional, you will play a vital role in providing person-centred support to our customers, helping them to reach their full potential and live fulfilling lives.
Key Responsibilities
- Support customers with complex needs, including those with moderate learning disabilities and autism, to develop their independence and social inclusion.
- Work collaboratively with colleagues and clinical teams to promote positive risk management and ensure the safety and wellbeing of customers and colleagues.
- Develop and implement person-centred support plans, taking into account the unique needs and preferences of each customer.
- Monitor and report on customer progress, identifying areas for improvement and developing strategies to address these.
- Participate in training and development opportunities to enhance your skills and knowledge in Positive Behaviour Support.
