About the Role

The Lancashire & South Cumbria NHS Foundation Trust is seeking a highly skilled and dedicated Senior Community Mental Health Practitioner to join our team. As a key member of our community mental health team, you will play a vital role in providing high-quality care and support to individuals with complex mental health needs.

Key Responsibilities

  • Manage a caseload of service users with a range of complex needs, developing and delivering evidence-based interventions and treatments.
  • Undertake team duty responsibilities, managing unplanned patient work and responding to escalation of risk/crisis situations.
  • Contribute to the learning and development of junior team members, and take a lead on/or contribute to projects identified by the Furness CMHT leadership team.

Requirements

  • Registered Mental Health Nurse Degree, Social Worker or Occupational Therapist.
  • Currently registered with a professional body.
  • Minimum of 18 months post-registered experience.
  • Experience of risk management, care coordination/managing a caseload of clients with a range of mental health needs.
