Care Support Specialist

Hilton Community Services Ltd is seeking a qualified individual to provide high-quality person-centred support to adults with learning disabilities.
The successful candidate will have excellent communication skills and be able to work independently as part of a dedicated team.
As a Care Support Specialist, you will play a vital role in ensuring that the people we support make their own choices and achieve their goals.
We offer a competitive salary of £30,000 - £35,000 per annum, depending on experience, plus benefits.
Key Responsibilities:
- Provide person-centred support to adults with learning disabilities
- Work collaboratively with the care team to develop and implement individual care plans
- Support individuals to access community resources and services
- Maintain accurate records and reports as required
Requirements:
- Relevant qualifications and experience in a similar role
- Excellent communication and interpersonal skills
- Able to work independently and as part of a team
- Physical fitness to meet the demands of the role
