Role Overview

NHS Scotland is seeking a highly motivated and proactive pharmacist to join our team in Wigtownshire. As a Clinical Pharmacist, you will play a key role in providing a clinical pharmacy service directly to patients, working closely with the multidisciplinary team to ensure safe and effective medicines management.

Key Responsibilities

  • Conduct clinical assessments and reviews of patients to identify areas for improvement in medicines management
  • Perform complex polypharmacy medication reviews with patients to optimize their treatment plans
  • Deal with medicines-related queries and provide expert advice to patients and healthcare professionals
  • Assess patients' ability to manage their medicines, including in their own homes, and provide support and guidance as needed
  • Contribute to the development and implementation of medicines management policies and procedures
