Primary Care Specialist

Are you a dedicated Advanced Clinical Practitioner seeking a new challenge in primary care? Primary Care 24 (PC24) is a leading not-for-profit social enterprise delivering high-quality NHS contracts exclusively. We are excited to share the opportunity for an Advanced Clinical Practitioner to join our Out of Hours Services team.
In this role, you will work independently with a high level of autonomy, assessing, diagnosing, and providing advice, care, and management to patients presenting with undiagnosed or undifferentiated illness via telephone consultations or face-to-face assessments for patients requiring home visits.
The key responsibilities include:
- Undertaking face-to-face and telephone triage consultations
- Interpreting pathology/radiology results/reports
- Presetting and reviewing medication
- Planning, implementing, and evaluating an effective management plan for the patient
About You
We are looking for an experienced ACP who can deliver quality individualized care to our patients with a high level of autonomy. The ideal candidate will have a strong background in clinical practice, leadership, and management, as well as education and research.
This is a permanent opportunity with part- and full-time options available. The salary range is £60,504 to £72,293 per annum plus unsociable enhancements, with a welcome bonus of £2,000 and retention bonus of £2,000. You will also receive an NHS pension scheme, 27 days annual leave, Continue Professional Development, Clinical Supervision, NHS car fleet scheme, Ride to work scheme, and Long service rewards.
