About the Role

We are seeking a highly skilled and motivated Adult Nurse to join our community team in a busy and well-established community setting. As a Community Health Nurse, you will be responsible for providing holistic care to patients in their own homes, promoting independence and choice in undertaking patient care.

Main Responsibilities
  • Conduct comprehensive assessments and develop individualized care plans for patients in their own homes.
  • Provide individualized treatment and promote self-care, working closely with patients and their families to achieve optimal health outcomes.
  • Develop and maintain extensive nursing skills, with access to excellent training opportunities and professional development.
  • Work collaboratively with multidisciplinary teams to deliver seamless health and social care services to patients in their own homes.
  • Stay up-to-date with the latest clinical guidelines and best practices in community nursing, ensuring high-quality care for patients.

Requirements
  • Registered Adult Nurse with NMC registration.
  • Experience of community nursing, with a strong understanding of the community nurse role.
  • Ability to demonstrate a wide range of clinical skills, including wound care, palliative care, and medicines management.
  • Car driver and access to a car for work purposes.
