About the Role

We are seeking a skilled and dedicated Adult Nurse to join our team in a busy community setting. As a Community Health Nurse, you will be responsible for providing high-quality, patient-centered care to individuals in their own homes.

Main Responsibilities
  • Conduct holistic assessments and develop personalized care plans for patients.
  • Provide individualized treatment and promote self-care to patients.
  • Develop and maintain extensive nursing skills through excellent training opportunities.
  • Participate in professional development to maintain area of practice and negotiate supervision.
  • Be aware of health, safety, and security risks.

Requirements
  • Registered Adult Nurse with NMC registration.
  • Experience of community nursing.
  • Understanding of the community nurse role.
  • Promote independence and choice in undertaking patient care.
  • Car driver and access to a car for work purposes.
  • Ability to demonstrate a wide range of clinical skills, e.g., wound care, palliative care, including syringe drivers, catheterisation, medicines management and administration, or willingness to develop skills through training.
NMC
  • Ability to demonstrate knowledge of NMC guidelines.
Community Experience
  • Demonstrates a commitment to working in the community setting.
Care
  • Ability to assess, develop, implement, and evaluate programmes of care.
