We are seeking a highly skilled and experienced Diabetes Specialist Nurse to join our team at Suffolk GP Federation CIC. As a key member of our diabetes service, you will play a vital role in providing high-quality care to patients with complex diabetes needs.

About the Role

The Diabetes Specialist Nurse will work as an autonomous practitioner, providing specialist clinics and highly specialized, evidence-based care to patients with diabetes across North East Essex. You will work closely with our Primary Care colleagues to deliver a holistic approach to diabetes management, including direct patient contact, mentorship, and audit using population health management and proactive care to enhance practice outcomes.

Main Duties and Responsibilities
  • Act as an advanced autonomous practitioner in your area of specialism, professionally and legally accountable for all aspects of work, including decisions about treatment and management of complex patients.
  • Maintain accurate and comprehensive documentation.
  • Hold a clinical caseload of those with complex needs and requiring advanced intervention, within a patient population using a high level of dexterity and accuracy in providing technical and clinical interventions aimed at improving services and patient outcomes.
  • Prescribe appropriate treatment as an Independent non-medical prescriber, and act as a specialist Practitioner recommending treatment regimens to optimize clinical outcomes in accordance with local policies.
  • Demonstrate advanced clinical reasoning and decision-making, and the ability to analyze complex facts or situations which may be conflicting and requiring appraisal of a range of options.
Person Specification

We are looking for a candidate with a dietetic or health-related degree or equivalent qualification, and current HCPC registration. Post-registration training in Diabetes, clinical supervisory skills, and non-medical prescribing qualification are desirable. Insulin pump training qualification or equivalent experience, and MSc in Diabetes or related subject, or willingness to work towards, are also highly valued.
