Job Summary

Midlands Partnership NHS Foundation Trust is seeking a highly skilled Pharmacy Technician to join our Medicines Optimisation team. As a key member of our team, you will play a vital role in ensuring the safe and effective use of medicines across our services.

Main Responsibilities

The successful candidate will be responsible for providing expert advice and support on medicines optimisation, working closely with our Integrated Care Pharmacist to ensure seamless pathways for medication support. You will work independently to provide a seamless pathway for medication support from in-patient to community services, developing prioritisation matrices, referral pathways, and undertaking structured medication reviews. You will also support the development of a comprehensive medicines optimisation service, working across health and social care interfaces to reduce the risk of harm from taking medicines and support procedures for medicines reconciliation, adherence, and self-administration.

About the Role

This is an exciting opportunity to join a dynamic team and contribute to the development of a truly integrated care framework across health and social care services. You will work closely with multidisciplinary teams, including Care Homes, GP Practices, and Primary Care Networks, to ensure safe transitions of care and reduce readmissions. You will also be responsible for identifying, developing, and providing support to meet medicines-related training needs for clinical and non-clinical staff.

Person Specification

The ideal candidate will have a BTEC and NVQ3 (or equivalent) in Pharmaceutical Science, be registered as a Pharmacy Technician with the General Pharmaceutical Council, and have experience in a broad range of hospital/community pharmacy activities, including dispensary management and clinical pharmacy activities. A nationally recognised qualification as an Accredited Checking Technician Certificate is desirable.
