We are seeking a highly skilled and compassionate Registered Nurse to join our team at East Coast Community Healthcare CIC. As a key member of our multidisciplinary team, you will be responsible for providing high-quality patient care and contributing to the development of our community healthcare services.
Key Responsibilities- Contribute to the assessment, planning, and implementation of patient care plans in collaboration with the multidisciplinary team.
- Provide holistic and person-centered care to patients with complex needs, including those requiring palliative care.
- Work closely with patients, families, and carers to promote independence and self-care.
- Participate in clinical supervision and contribute to the development of clinical governance policies and procedures.
- Collaborate with colleagues to develop and implement care pathways and protocols.
- Contribute to the evaluation of training programs and the development of education and development opportunities for staff.
- Registered Nurse qualification and current NMC registration.
- Experience of working in a community healthcare setting, preferably in a palliative care or intermediate care environment.
- Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and colleagues.
- Ability to prioritize and manage workload, with a focus on delivering high-quality patient care.
- Knowledge of clinical governance policies and procedures, including CQC fundamental standards.
- Ability to work flexibly, including evenings, weekends, and bank holidays.
