Job summary

We have an exciting vacancy for a Registered Nurse (band 5) based in Beccles, Suffolk in our Intermediate Care Centre (ICC) providing rehabilitation and Specialist Palliative Care services to patients in the Great Yarmouth and Waveney area.

Our aims are to reduce acute hospital admission and facilitate early discharge for our patients by utilising the skills of the multidisciplinary, skilled team of nurses and therapists permanently attached to the centre.

The ICC also has 6 consultant led Specialist Palliative Care beds which provide intensive symptom control and management to allow patients to maintain the best quality of life possible.

The post holder will provide a high standard of care as part of a multi-disciplinary team to patients within the intermediate care service. The post holder will be working as an autonomous practitioner applying a high level of clinical reasoning to ensure that individualised care is delivered safely.

The postholder will work hour shifts, including weekends and bank holidays.

Main duties of the job

The role of the Registered Nurse is to work with the multi professional team and the Nurse in Charge contributing to the on-going assessment, providing and monitoring care in Line with the service pathways and as agreed with supervising clinician(s).

They will monitor the condition and health needs of people within their care on a continual basis in partnership with the multi-professional team.

The Nurse has a breadth of knowledge across the lifespan, providing holistic and person centered care and support for people of all ages and in a variety of settings.

Registered Nurses are equipped with the knowledge, understanding, skills, attitudes and

behaviours relevant to employment; and will act in accordance with the NMC Code:

Professional standards of practice and behaviour for nurses, midwives and nursing associates.

Following the announcement of a Agenda for Change pay award, this role will be eligible for a salary increase effective in October 2024 pay. Details of the new pay structure can be found on the NHS Employers website.

The Unit comprises of 20 intermediate care beds delivering rehabilitation and 6 Specialist Palliative Care (SPC) beds in a purpose designed setting. For our intermediate care patients our aims are to reduce acute hospital admission and facilitate early discharge by utilising the skills of the multidisciplinary team of nurses and therapists. For the SPC patients we aim to provide holistic symptom management.

Job responsibilities

Contribute to, support and promote ECCHs, strategic direction, values and culture in relation to proactive and reactive services.

Discuss all treatment options with sensitivity, knowledge and expertise, and to act as a patient advocate when appropriate, respecting patient confidentiality with privacy and respect for diverse cultural backgrounds and requirements

Actively manage the patients admission, involving clinical experts, when relevant, to advise in the treatment plan. Proactive discharge management, ensuring patients are consulted and proper management of situations where patients need assistance in decision making.

Act as a resource for other health and social care professionals on matters relating to nursing.

Proactive participation in multidisciplinary team meetings

Take charge of the service and/or site in the absence of a Sister/Charge Nurse, accepting responsibility during this time to lead the team and maintain a safe environment

To act as an effective role model to other staff, being clear about expectations and ensuring that safe patient care is central to all activities

To provide complex information to the patients, clinicians and other agencies to assist in decision-making, using persuasive, motivational, negotiating, training, empathic and re-assurance skills

Participate in clinical supervision within the team as required.

Participate and maintain a learning environment and maximise opportunities for education and development in the clinical area.

Contribute to the clinical governance agenda through participation in clinical risk assessment and clinical audit.

Contribute to an environment conducive to effective working, respecting and supporting staff to deliver high quality clinical services.

Plan and organise a range of complex clinical activities to ensure best practice is delivered to patients in your delegated area of responsibility.
