About the Role:

This Band 6 position offers the opportunity to work as a Community Dietitian within the North East London NHS Foundation Trust. As a member of our team, you will be responsible for providing specialist dietetic expertise to adult patients in their homes, clinics, and community hospitals.

Main Responsibilities:

  • Deliver high-quality patient care through assessment, planning, implementation, and evaluation of individualized nutrition plans.">
  • Collaborate with multidisciplinary teams to ensure seamless patient care and transitions.">
  • Develop and implement evidence-based nutrition programs to improve patient outcomes.">
  • Conduct home visits and assessments to identify patient needs and develop personalized nutrition plans.">
  • Provide education and training to other healthcare professionals on nutrition-related topics.">

Requirements:

  • Experience working in community or clinical settings (Band 5 or above).">
  • Strong passion for promoting public health and nutrition.">
  • Valid driver's license and access to a vehicle.">
  • Excellent communication and interpersonal skills.">
  • Certification as a Registered Dietitian or equivalent qualification.">
