Nurse Care Coordinator

This position requires strong clinical skills, excellent communication, and a passion for delivering patient-centred care in a community setting. You will work collaboratively with other healthcare professionals to ensure coordinated care and support the holistic needs of patients with acute, chronic, or complex health conditions.
Key Responsibilities:
- Clinical Care
- Deliver a range of nursing interventions, including wound care, medication administration, catheter care, and end-of-life care.
- Conduct holistic assessments of patients' physical, emotional, and social needs to develop and implement individualised care plans.
- Monitor and evaluate patients progress, adapting care plans as necessary to achieve the best outcomes.
- Provide health education and advice to patients and carers to support self-management and promote independence.
- Community-Based Care
- Work autonomously in patients homes and community settings, ensuring safe and effective care delivery.
- Manage a daily caseload, prioritising tasks and responding to changes in patients conditions.
- Use digital tools and systems to plan, record, and report on care delivery in line with organisational policies.
- Team Collaboration
- Work closely with other healthcare professionals, such as GPs, therapists, social workers, and voluntary sector partners, to ensure coordinated care.
- Participate in multidisciplinary team meetings and case conferences to support care planning and review.
- Act as a patient advocate, ensuring their preferences and needs are central to care planning and decision-making.
- Professional Development
- Commit to continuing professional development (CPD) to maintain and enhance clinical skills and knowledge.
- Participate in clinical supervision, reflective practice, and training sessions provided by the organisation.
- Support the development of students, healthcare assistants, and less experienced colleagues through mentoring and guidance.
- Compliance and Governance
- Maintain accurate and timely clinical records in accordance with professional and organisational standards.
- Adhere to local and national policies, guidelines, and regulations, including infection control and safeguarding protocols.
- Promote and maintain a culture of safety, quality, and continuous improvement.
Salary and Benefits:
The estimated salary for this position is £42,000 - £58,000 per annum, depending on experience.
