About the Opportunity:

We are seeking an experienced and dedicated Nurse to join our team as a Nurse Practitioner - Community Care. This role involves providing high-quality nursing care to patients in their own homes or community settings, supporting them to maintain independence and achieve their healthcare goals.

Key Responsibilities:

  • Clinical Assessment:
    • Conduct comprehensive assessments of patients' physical, emotional, and social needs to develop and implement individualised care plans.
    • Monitor and evaluate patients progress, adapting care plans as necessary to achieve the best outcomes.
  • Community-Based Care:
    • Work autonomously in patients homes and community settings, ensuring safe and effective care delivery.
    • Manage a daily caseload, prioritising tasks and responding to changes in patients conditions.
  • Team Collaboration:
    • Work closely with other healthcare professionals, such as GPs, therapists, social workers, and voluntary sector partners, to ensure coordinated care.
    • Participate in multidisciplinary team meetings and case conferences to support care planning and review.
