Job Summary

We are seeking a highly skilled and experienced Senior Clinical Nurse to join our Learning Disability Team in Plymouth. As a key member of our multidisciplinary team, you will be responsible for assessing, planning, and managing the care needs of individuals with complex learning disabilities.

Main Responsibilities

  • Participate in health needs reviews and health assessments to identify needs and plan care accordingly.
  • Identify, plan, and manage risk of an individual's care needs, relating to physical health, behavioral, and mental health.
  • Be responsible for assessing, planning, and making recommendations for future care in partnership with patients, carers, and multi-professional agencies.
  • Liaise with primary and secondary care agencies to ensure better health outcomes for individuals.
  • Work within the Mental Capacity Act and evidence best interest decision making.
  • Maintain effective and appropriate communication networks with members of the multidisciplinary team, the independent sector homes, and all stakeholder agencies and organizations.
  • Contribute to multi-disciplinary team specialist assessments.
  • Ability to work as an autonomous practitioner, planning, prioritizing, and organizing workload according to the needs of the service.
  • Provide any relevant information and explanation to clients and carers at a level commensurate with their ability to understand using total communication.
  • Provide evidence-based advice and guidance to care staff on the management of health needs to patients and signpost to other services.
  • Act as a supervisor and/or assessor for students demonstrating high standards of practice and professional conduct.
  • Undertake health promotion/education whenever possible to all adults with LD and also to contribute to planned health promotion activities.
  • Work with all other agencies with regards to the safe guarding process.
