About the Role

We are seeking an experienced Clinical Pharmacist to join our team at Alliance for Better Care CIC. As a Clinical Pharmacist, you will play a vital role in supporting our Primary Care Network in delivering high-quality patient care.

Key Responsibilities

  • Provide clinical leadership on medicines optimisation and quality improvement initiatives
  • Manage and review patient medication lists, identifying opportunities for improvement and implementing changes as necessary
  • Work closely with GPs, nurses, and other healthcare professionals to ensure seamless care coordination
  • Develop and implement education and training programs for healthcare staff on medicines optimisation and patient safety
  • Contribute to the development and implementation of local and national guidelines on medicines use
  • Collaborate with hospital colleagues to ensure continuity of care for patients with complex needs
  • Provide expert advice on medicines-related issues to patients, carers, and healthcare professionals
  • Participate in multidisciplinary team meetings to discuss patient care and develop care plans
  • Contribute to the development and implementation of quality improvement initiatives to improve patient outcomes

Requirements

  • Registration with the General Pharmaceutical Council
  • Completion of an undergraduate degree in pharmacy and registration with the General Pharmaceutical Council
  • Completed CPPE course
  • Experience in primary care or a related field
  • Strong communication and interpersonal skills
  • Ability to work independently and as part of a team
  • Knowledge of medicines optimisation and patient safety principles
