Senior Clinical Pharmacist

This is an exciting opportunity for an experienced pharmacist to support a wide range of patients of all ages in a patient-facing role. The successful candidate will work as part of a multi-disciplinary team in a primary care setting, taking responsibility for areas of chronic disease management and undertaking clinical medication reviews to proactively manage patients with complex polypharmacy.
Main Duties of the JobThe successful candidate will provide support to general practice staff with regards to prescription and medication queries, help manage the repeat prescribing system, and deal with acute prescription requests. They will also provide expertise in clinical medicines advice, address both public and social care needs of patients, and contribute to the delivery of the local medicines optimisation strategy.

Job Responsibilities- See patients in multimorbidity clinics and implement improvements to patients' medicines, including de-prescribing.
- Manage own caseload and run long-term condition clinics, prescribing as an independent prescriber for conditions where medicines have a large component.
- Undertake clinical medication reviews with patients with multimorbidity and polypharmacy, implementing own prescribing changes and ordering relevant monitoring tests.
- Provide patient-facing clinics for those with questions, queries, and concerns about their medicines.
- Act as a source of medicines information for the practice team and patients, suggesting and recommending solutions.
- Reconcile medicines following discharge from hospitals, intermediate care, and into care homes, identifying and rectifying unexplained changes.
- Manage own caseload of patients with common/minor/self-limiting ailments, signposting to community pharmacy and referring to GPs or other healthcare professionals where appropriate.
- Ensure that patients are referred to the appropriate healthcare professional for the right care, reducing workload for other clinicians.
- Work with case managers, multidisciplinary review teams, hospital colleagues, and virtual ward teams to manage medicines-related risk for readmission and patient harm.
- Produce and implement a practice repeat prescribing policy, managing the repeat prescribing reauthorisation process, and ensuring patients have appropriate monitoring tests in place.
- Identify national and local policy and guidance that affects patient safety through the use of medicines, implementing changes to medicines and guidance for practitioners.
- Take a leading role in the contractual requirements to support frail patients with their medication, including medication review and patient identification.
- Work with CCG Medicines Management (MM) Team to implement local and national guidelines, liaising with hospital colleagues where prescribing needs to be returned to specialists.
- Provide education and training to primary healthcare team on therapeutics and medicines optimisation, providing training to visiting medical, nursing, pharmacy, and other healthcare students where appropriate.
- Work with the general practice team to ensure the practice is compliant with CQC standards where medicines are involved, undertaking risk assessment and ensuring compliance with medicines legislation.
- Support public health campaigns, providing specialist knowledge on all public health programmes available to the general public.
- Manage own caseload of vulnerable housebound patients at risk of hospital admission and harm from poor use of medicines, implementing own prescribing changes and ordering monitoring tests.
- Work with colleagues to identify and provide leadership on areas of prescribing and medicines optimisation, conducting clinical audits and medicine optimisation projects.
- Ensure that patients are referred to the appropriate healthcare professional for the right care within an appropriate period of time, pathology test results, common/minor ailments, acute conditions, long-term condition reviews, etc.
