Job Summary

We are seeking a highly motivated and compassionate Support Practitioner to join our Moray Relief Pool team. As a Support Practitioner, you will play a vital role in providing high-quality care and support to individuals in their own homes, promoting their independence, dignity, and well-being.

Key Responsibilities

  • Provide person-centered care and support to individuals with disabilities and additional support needs, promoting their independence and dignity.
  • Assist with daily living tasks, such as personal care, cooking, and housework, as required.
  • Support individuals to engage in activities and hobbies that promote their physical and mental well-being.
  • Develop and maintain positive relationships with individuals, their families, and other stakeholders.
  • Contribute to the development and implementation of individual support plans, ensuring that the needs and preferences of individuals are met.

Requirements

  • A caring and compassionate attitude, with a commitment to promoting the dignity and well-being of individuals.
  • Excellent communication and interpersonal skills, with the ability to build positive relationships with individuals, families, and other stakeholders.
  • Ability to work flexibly, including evenings, weekends, and public holidays, as required.
  • Confidence in the use of computers and other technology, with the ability to maintain accurate records and reports.
  • A positive approach to learning, with a commitment to ongoing professional development and training.
