Our Team in Wiltshire

We are looking for a qualified social worker to join our ongoing support team in Wiltshire. The successful candidate will work alongside a multidisciplinary team, including social workers, occupational therapists, and social care practitioners.

Our team provides ongoing support to adults with disabilities or health conditions, and we are seeking a dedicated individual to join us in this vital work.

Key Responsibilities

  1. Complete Care Act Assessments with adults in need of support
  2. Develop and implement support plans to meet individual needs
  3. Work collaboratively with other professionals to ensure effective care and support
  4. Provide advocacy and guidance to individuals and their families
  5. Contribute to the development of our ongoing support team

About the Role

This is a challenging and rewarding role that requires a strong understanding of adult social care principles and practices. The successful candidate will have excellent communication and interpersonal skills, and be able to work effectively in a fast-paced environment.
