Experienced Social Worker, Learning Disability, Adult Social Care

We are seeking an experienced social worker to join our Learning Disability Team, supporting individuals to meet their needs and live independently. As a key member of our team, you will work closely with residents, families, and professionals to provide high-quality care and support.
Key Responsibilities- Manage a caseload of complex cases, conducting Care Act Assessments, reviews, and support planning.
- Collaborate with health, provider services, safeguarding, and mental health colleagues to ensure comprehensive support.
- Provide supervision and support to colleagues, managing safeguarding enquiries and participating in the duty rota.
- Stay up-to-date with best practices, policies, and procedures, applying a strength-based approach to support individuals.
