Complex Needs Navigator

Job Summary: We are seeking a dedicated and compassionate Complex Needs Navigator to join our team at Two Saints. As a key member of our outreach team, you will provide personalized support to individuals experiencing homelessness, helping them to access essential services and achieve their goals.
Main Responsibilities:
- Provide assertive outreach support to a small caseload of complex rough sleepers, maintaining a consistent and persistent approach.
- Engage with rough sleepers on the street, working in partnership with health, substance misuse, criminal justice, and local authority agencies.
- Manage a caseload of clients, maintaining accurate records of support offered, actions to be completed, and outcomes achieved.
- Complete initial assessments to identify client needs and produce person-centered outcome and holistic support plans.
- Identify positive actions and small successes to create change.
- Create dynamic risk assessments with a flexible and creative approach.
- Provide information and support on housing pathways and advocacy for clients.
- Liaise with statutory and voluntary agencies on behalf of clients.
- Accompany clients to interviews and appointments.
- Help reduce time spent on the street, leading to reduced contact with the police and other emergency services.
- Provide emotional support in crisis situations, reducing stress and anxiety.
- Support with practical skills development, including managing a UC claim, shopping, probation, and health appointments.
- Support to maintain environment to avoid eviction if accommodated.
- Provide in-reach support to those placed in emergency accommodation, including bed & breakfast.
- Attend community meetings and presentations with managers to support additional funding for the role.
- Maintain accurate records, including databases to support managers to provide data and information to commissioners and funders.
- Support with statistical reporting mechanisms, such as street counts and responding to street link reports.
- Access personalization fund creatively to identify opportunities and achieve outcomes.
- Support the whole team and take an active part in reflective practice.
- Develop and maintain excellent relations with the local community.
Requirements:
- Previous experience in this type of work is essential, with a real passion for working with homeless individuals.
- Clear verbal and written English.
- Good IT and keyboard skills.
- Polite, assertive manner, ability to self-motivate.
- Passion for working in a client/customer-facing role.
- Knowledge of the benefits system and tenancy legislation.
- Interest and genuine concern for homelessness and related issues.
- Patience, empathy, and understanding towards entrenched behaviors.
- Ability to work flexibly and creatively surrounding rotas to meet the needs of clients.
- Knowledge of voluntary and statutory agencies.
- Full driver's license and access to vehicle.
- Flexibility with shift times to suit the needs of the service.
