The Role

As a Care Team Leader, you will be responsible for leading and supervising your shift, case managing residents, and liaising with families and healthcare professionals to ensure the best care plans are created and delivered. You will also be responsible for administering and recording medication, carrying out risk assessments, and working closely with the leadership team to develop services.

Some of the key responsibilities of this role include:

  • Leading and supervising your shift to ensure the highest standards of care are maintained
  • Case managing residents and their families to ensure their needs are met
  • Administering and recording medication in accordance with company policies and procedures
  • Carrying out risk assessments to identify potential risks and take steps to mitigate them

We are looking for a candidate who is well-organised, IT literate, and able to allocate and delegate work effectively. You should have a proven ability to lead and supervise others, a good understanding of care planning processes, and experience in administering medication.
