As an LPN, you will play a vital role in delivering high-quality patient care, working collaboratively with RNs and other healthcare professionals to achieve optimal outcomes.

About the Role

Your responsibilities will include assessing patients' needs, developing individualized care plans, and providing evidence-based interventions. You will work closely with our multidisciplinary team to deliver coordinated care, participating in controlled substance counts, and ensuring adherence to safety procedures.

Key Responsibilities
  • Conduct comprehensive assessments of patients' physical, emotional, and social needs, identifying areas for improvement and developing targeted interventions.
  • Implement treatment programs under the guidance of RNs, incorporating medication administration, IV therapy (if certified), and diagnostic testing according to established protocols.
  • Collaborate with multidisciplinary teams to deliver coordinated care, document patient interactions using SOAP charting, and communicate effectively with colleagues and stakeholders.
  • Participate in ongoing education and training to enhance skills and knowledge, maintaining accurate records and adhering to policies and procedures.
