Job Title: Care Manager - Adult Social Support

At All Saints Support Health Care Agency, we are seeking an experienced Care Manager - Adult Social Support to lead our dedicated team in delivering person-centred care that promotes independence and well-being.

Key Responsibilities:

• Proven experience in adult social care management or a similar role
• Experience in writing and updating care plans, identifying, managing, and updating risks
• Ability to work closely with clients, families, and partner organisations to ensure that adults in the community receive the support they need to live fulfilling, independent lives

Requirements:

• Proven leadership skills to inspire a team and deliver high-quality care
• Ability to liaise effectively with Social Workers, Commissioners, and other stakeholders
• Knowledge of the Stockport area and local community
