Care Management Specialist - 12 Month Fixed Term Contract

North Staffordshire Combined Healthcare NHS Trust is a leading provider of mental health, social care, learning disability, and substance misuse services in the West Midlands.

This development presents a unique opportunity for motivated and enthusiastic qualified nurses (primarily learning disability/mental health professionals) to join the TCP Case Management Team.

The Case Management team works within the Trust to support the delivery of Transforming Care across all Trust Divisions, working to ensure that people with learning disabilities and/or autism are supported to lead healthy, valued lives.

Main Responsibilities

The purpose of this role is to support the commissioning of safe and effective services for people with a learning disability, supporting discharge from hospital and oversight of commissioned community packages of care.

This involves participating in assessment, planning, and implementation of pro-active care and case management for the population of Staffordshire and Stoke-on-Trent with a Learning Disability and/or autism.

The post holder will be a member of the Learning Disability Transforming Care Team within the Specialist Directorate of North Staffordshire Combined Healthcare NHS Trust.

Key Requirements
  • Significant experience of post-qualification work in core professions, working collaboratively with multi-agencies/professionals, working with young people and/or adults with learning disabilities and/or autism with complex behavioural and/or mental health issues.

Estimated salary: £57,500 per annum.
