Job Overview

We are seeking a highly skilled Complex Care Specialist to join our team at North Staffordshire Combined Healthcare NHS Trust. As a leading provider of mental health, social care, learning disability, and substance misuse services in the West Midlands, we are committed to delivering high-quality care that meets the unique needs of our service users.

The successful candidate will work closely with our Transforming Care Team to support the commissioning of safe and effective services for individuals with learning disabilities and/or autism. This role is ideal for a motivated and enthusiastic qualified nurse or professional who is passionate about making a positive impact on people's lives.

Key Responsibilities
  • To assess, plan, and implement proactive care and case management for individuals with learning disabilities and/or autism in Staffordshire and Stoke-on-Trent.
  • To collaborate with multi-agencies and professionals to ensure seamless care delivery and minimize hospital admissions.
  • To participate in the development and implementation of strategies to improve patient outcomes and reduce healthcare costs.
Requirements

We are looking for an individual with:

  • Significant experience working with young people and adults with learning disabilities and/or autism, particularly those with complex behavioral and/or mental health issues.
  • A strong understanding of Transforming Care principles and their application in practice.
  • Excellent communication and interpersonal skills, with the ability to build strong relationships with colleagues, patients, and families.
