Complex Care Healthcare Assistant Job Description:

Location: Stoke-on-Trent

Salary: Up to £16.00 per hour

About the Role:

This challenging yet rewarding role requires a compassionate and skilled Adult Care Specialist to support individuals with complex care needs in their own homes.

Main Responsibilities:

  • Care planning and delivery: Work closely with our Nurse Managers to develop and implement personalized care plans.
  • Health monitoring: Regularly monitor clients' health conditions and report any changes or concerns.
  • Collaboration and communication: Maintain effective relationships with clients, families, and healthcare professionals.
