About the Role

We are seeking a skilled and proactive Female Support Worker to join our team in supporting a lady with a brain injury in the South Shields area.

The successful candidate will be responsible for assisting the client with her rehabilitation programme, increasing her independence, and providing support with community activities.

This is a rewarding opportunity to make a positive impact on someone's life and work in a dynamic and supportive environment.

Key Responsibilities
  • Assist the client to exercise choice and control over their own lives as far as they are able in everyday matters.
  • Liaise with and support the family as required.
  • Assist the client with personal care as needed.
  • Support the client to use local amenities such as shops and leisure facilities and attend community events.
  • Follow the service user plan as documented for the client.
  • Maintain accurate and confidential records.
  • Participate in manual handling procedures as specified for the client.
  • Maintain professional boundaries with the client and their family members, adhering to direction from the case manager as appropriate.

  • You are reliable and professional, with a strong commitment to honesty and discretion.
  • You are friendly and approachable, with excellent communication skills.
  • Availability and capability to work hours in rota.
  • You are flexible and able to work well under pressure.

About Us

Active Care Group provides an essential service to both adults and children who have suffered brain and spinal injury. Our team of qualified staff assess clients' needs and offer individually designed case management and rehabilitation programmes.

We are a successful rehabilitation and case management consultancy, working in close co-operation with the legal profession. We now have four offices employing around 80 staff, managing over 400 support workers and providing services to just under 400 clients nationally.
