About the Role
We are seeking a highly motivated and experienced Social Worker to join our Mental Health Team at Eden Brown Synergy.
As a Social Worker in this role, you will work closely with a multidisciplinary team to provide high-quality care and support to service users.
- Manage a complex caseload and be responsible for social care assessments, risk assessments, and facilitating group activities.
- Contribute to ICR meetings, supporting service users to make viable plans for their future.
- Educate hospital staff about relevant safeguarding matters.
- Complete a full annual social care reassessment within the CPA format.
- Facilitate access to welfare rights, housing, advocacy services, and associated support services.
Requirements
- Registration with Social Work England.
- Social Work degree.
