As a Shared Lives Officer, you will be responsible for assessing, monitoring, and developing Carers to ensure they provide excellent support to adults in their care.
You will work within a dedicated team to ensure that Shared Lives Carers meet Care Quality Commission (CQC) and in-house performance standards.
Key responsibilities include:
- Assessing and monitoring Carers to ensure they meet the needs of adults in their care.
- Developing and implementing care plans to support adults in leading independent and fulfilling lives.
- Working with the Referrals Officer to coordinate new placements and ensure a robust matching process.
- Supervising Shared Lives Assistant Officers and Shared Lives Workers.
Requirements:
- Degree in Social Work or equivalent in a related discipline.
- Experience of working with people with assessed social care needs.
- Knowledge of relevant Social Care legislation.
- Good computer literacy and report writing skills.
- Excellent interpersonal skills and mediation and negotiation skills.
- Registration with the Health Care Professionals Council.
