About the Role

We are seeking a skilled Community Registered Nurse to join our Easington North Community Intervention Team. As a key member of our team, you will be responsible for providing high-quality patient care and implementing the mental health transformation framework.

Your primary duties will include acting as a named nurse or key worker for patients, ensuring care plans are based on current risk assessments and evidence-based practice. You will also be responsible for providing health education and advice on health promotion to patients and their carers.

As a Community Registered Nurse, you will contribute to the planning and coordination of clinical interventions to challenging behavior such as self-harm and aggression. You will work collaboratively with service users and their families/carers, as well as other agencies, to provide comprehensive care.

To be successful in this role, you will need to have current professional registration with the Nursing & Midwifery Council, as well as experience working with people with mental ill health/learning disabilities. You will also need to be trained in Clinical Risk Assessment and Management and have a confident working knowledge of clinical medicines management.
