Community Mental Health Nurse

We are seeking a skilled and compassionate Band 5 Community Registered Nurse to join our Easington North Community Intervention Team.
The successful candidate will be responsible for providing high-quality care to patients with mental health needs, working collaboratively with multidisciplinary teams to develop and implement individualized care plans.
Key responsibilities will include:
- Acting as named nurse or key worker for patients, taking responsibility for implementing the mental health transformation framework.
- Ensuring care plans are based on current risk assessment, evidence-based practice, and whole system support requirements.
- Undertaking risk assessments and devising and implementing actions and care plans that take the risk formulation fully into account.
- Providing health education and advice on health promotion to patients and their carers.
