Job Title: Nurse Specialist - Children's Continuing Care

We are seeking a qualified and enthusiastic Children's Nurse to join our team as a Band 5 Nurse Specialist in Children's Continuing Care. This is an exciting opportunity to work with a dedicated team providing specialist care to children and young people with complex health needs in the community setting.

Main Responsibilities:
  • Provide high-quality, integrated assessment and ongoing support to individual children with complex health needs.
  • Work collaboratively with the team to develop and implement bespoke care packages for children and young people.
  • Use manual handling and personal protective equipment (PPE) as required for this role.
  • Act as a Key Worker for individual children, providing support and guidance to families and carers.
Requirements:
  • Qualified Children's Nurse with a passion for working with children and young people with complex health needs.
  • Ability to work flexibly, including nights, weekends, and bank holidays.
  • Excellent communication and interpersonal skills.
  • Ability to work as part of a team and provide support to colleagues.
